I've had nightmares shooting over cars and poking a camera out the window to take a shot.

I find that for the most part, it doesn't matter if it is or it isn't running.
That heat stays around for some time after turning the car off.
I have a black SUV, there are heat waves coming off of it a year long.
Even with the car not running, if you jumped out and started to shoot over the hood, you'll probably get heat waves.
I've had it in the winter where I've gotten out of my warm car with my camera pointing down, and because of the lens hood
holding in some warm air, I'd get heat waves when I brought it up to shoot.
Sometimes it's just humidity in the atmosphere.
Maybe that's a better way of putting it, that I think that some kind of atmosphere is the reason the image is slightly off.

I am not suggesting for heat but rather for vibrations that run through the camera and lens. It is always recommended to shut off the vehicle engine to prevent vibration.

Car was running, but I think I also got out of the car and took a few more. I can't remember. I think it's just all technique. The 1.4 with GM is cable of producing very sharp pictures when I'm closer. I just can't get it all that sharp the farther I'm from the subject. I have fairly steady hands.

I have used the same setup as you are, and have gotten the same results, but not so much with standing subjects.
I have issues with birds flying and encountering this.
I've tried it with a 1.4 tc and a 2x tc, pretty much the same thing. A 10 shot burst will give me 2 shots dead on, and 8 shots slightly off.
People have told me that it has to do with the camera only adjusting the focus 5 times a second, while I'm shooting 10 frames a second.
I think it still could be atmospheric, but I'm not positive. (having used that camera and lens setup)
Maybe try some different setting for the AF. Tracking on and off. The different settings for the tracking modes.
If you figure it out, I'd like to hear what you came up with.

Looking at a wading Great Egret in breeding plumage with radiating ripples in the water below the beak; 3 drops of water are on the underside near the tip of the Egret's beak. I suspect that a drop of water fell off the beak to create the radiating ripples.
